Birth Haven LLC operates a single location at 141 Main Street, Ste 1, Salem, NH 03079. This page describes the contact forms on this website only. It does not describe how medical records, clinical care, or protected health information are managed. The information you choose to share

The website offers two forms: a general inquiry form on the Contact page and a tour request form on the Book a Tour page. You choose what to enter. The fields you see on those forms are the fields involved:

• Full name, so the team knows who they are speaking with.
• Email address, so the team can reply to your message or tour request.
• Phone number, so the team can follow up by phone.
• What we can help you with (inquiry type) and how you heard about us (referral source) on the Contact form.
• How you heard about us (referral source) on the tour request form.
• An optional message on the Contact form about your inquiry.
• An optional estimated due date on the tour request form, which you may share if you are currently pregnant and know it.

Both forms ask you not to include sensitive medical information in your message. For health-related questions, please contact the office directly rather than through these general website forms.

How a submission is handled

When you submit either form, the website sends the details you entered as a lead event to the practice customer relationship management (CRM) system so the Birth Haven inquiry team can review your message or tour request and follow up with you. The event includes the form fields above along with basic visit details such as the page address and the type of device and browser you are using. The on-screen confirmation does not confirm delivery to the team. If you do not hear back, please contact the office.

Links and third-party services

This website links to outside services that have their own privacy practices. For example, the directions link opens Google Maps, and any donation or community pages may link to a separate donation provider. Once you leave this website, the privacy practices of those other services apply. Birth Haven does not control how those services handle your information.
